Mercy’s Place intake process starts with a prospective client completing the Contact Form on our website. Our clinical support staff will contact the client to gain any additional information/inquiry and what providers we have available. If the potential client decides to proceed with an available provider, they will be sent intake forms to be completed. When forms are completed, they will be referred to their provider and the provider will contact them to schedule their initial appointment.
